Deconstructing Demons:The Case of Geoffrey

        Editor's note: This module is organized around a 14-session therapy case study of “Geoffrey,” as conducted by Dr. Jan Mohlman and as conceptually developed and written up by her research team. An African-American man of 66, Geoffrey presented with “a complex range of psychiatric conditions ( e.g., major depressive disorder, social anxiety disorder, and a history of polysubstance dependence); . . . challenging life circumstances (e.g., limited financial and social resources, and poorly controlled medical illness); . . . and impairment in several cognitive domains (attention, memory, executive functions).”  Because of this complexity, Dr. Mohlman employed a transdiagnostic model in the therapy, including both cognitive-behavioral and executive-skills-training interventions, all the while striving to balance between empirically-supported and individualized approaches to treatment design.

        The case of Geoffrey raises a variety of general issues – for example, how to adapt methods developed on younger patients to older clients; the balance between manualized and individualized therapy; the interplay between cognitive-behavioral therapy and executive skills training; and cultural issues (specifically, bridging the gap between a therapist who was young, white, female, and professionally and economically successful, and a patient, as mentioned above, who was an elderly, African-American male with very limited economic resources and modest vocational accomplishments). 
 
         These issues are impressively explored in commentaries by Dr. Bob Knight and Dr. Lee Hyer, both leading experts in psychotherapy with the elderly. Dr. Knight’s commentary  focuses on the advantages of adding executive skills training to improve cognitive functioning in older adults and to enhance responsiveness to CBT; the importance of therapist flexibility in working with older adults; and the influence of cohort differences and ethnic differences in the elderly. Dr. Hyer’s commentary relates Geoffrey’s case study to the broad context of aging as it is associated with clinical problems of depression, anxiety, and cognitive impairment; the nature and goals of evidence-based practice and accountability as they apply to real-world therapy practice with the elderly; and common factors in treatment that the case of Geoffrey illustrates.  The series concludes with a thoughtful response to the commentaries by Dr. Mohlman and her team.
